Premature ejaculation (PE) is one of the most frequent sexual disorders in male persons (along with erection problems). It causes an expressed psycho-traumatic effect on both sexual partners, which often leads to a break in relations. Depending on the causes of the ailment, healthcare professionals select the optimal treatment: from conservative methods to surgical treatment. In this article, we will tell you about the definition and causes/triggering factors of this disease. In addition, you will learn how to stop premature ejaculation.
Types and causes of premature ejaculation
Premature ejaculation is divided into primary, observed from the very beginning of sexual activity, as well as secondary, having acquired character and caused by some other disease. Primary early ejaculation is associated with the peculiarity of the functioning of the brain, namely its structures, regulated by the level of a biologically active substance – serotonin. It has been found that in men suffering from this form of PE, there is an increased activity of the receptor apparatus responsible for the re-uptake of this substance. This, in turn, leads to a low concentration of serotonin and an increase in the activity of the cerebral ejaculation center. Similarly, the primary form of the disease is associated with a congenital hypersensitivity of the glans penis. Its skin in some men is excessively filled with sensitive receptors. Secondary PE is often caused by chronic prostatitis (changed semen color can be a sign of this disease). In addition, the list of its possible causes includes:
1) excessive sexuality of a man and his extremely strong sexual constitution: a man quickly recovers after sex, but his sexual intercourse does not last very long;
2) deterioration of the regulation of normal ejaculation directly in the sacral-lumbar spine with infringement of the spinal cord (which leads to osteochondrosis of the lower spine, various vertebral hernias, etc.);
3) inflammatory processes, occurring in colliculus seminalis, anatomical structures where the human reflex of ejaculation originates (inflammation can be caused by sexual infections);
4) diseases of the thyroid gland, accompanied by an increase in its function (“hyperthyroidism” according to medical terms);
5) taking medications or psychoactive substances that affect the process of ejaculation (for example, marijuana);
6) psychological problems (due to experiences related to sex).
How to stop premature ejaculation
How to treat the disease? What methods have been developed for this purpose? The first and only selective serotonin re-uptake inhibitor, which is designed to treat PE, is dapoxetine. Currently, it is available in Mexico, many European and Asian countries. The application for approval of the drug in the US is now being processed by the FDA, and it is expected that the drug will soon appear in the US market. It is important to know that dapoxetine premature ejaculation pills very quickly reach an effective concentration in the blood and practically have no psychotropic effect. It is taken 1-3 hours before the planned sexual activity and the effect of taking it are felt only one-time. Dapoxetine allows you to increase the duration of the sexual act by several times compared with going without it. It should also be noted that the medication is quite effective in patients with premature ejaculation, which occurs before the penis enters the vagina.
In patients with a secondary form of PE associated with erectile dysfunctions, phosphodiesterase-5 inhibitors have the proper therapeutic effect. Currently, in the Russian market, there are several drugs for the cure of erectile dysfunction: Viagra, Cialis, Levitra, Stendra, and others. All these agents showed high clinical efficacy in patients with erectile dysfunction, approximately equal to 70-90%. In patients with PE, which is not associated with erectile dysfunction, the use of phosphodiesterase-5 inhibitors is ineffective.
In patients with a secondary form of early ejaculation associated with an increase in thyroid function (hyperthyroidism), treatment is carried out by an endocrinologist. Most often, thyreostatic therapy is recommended with thiamazole, propylthiouracil or radioactive iodine. In a number of situations, surgical treatment is performed. Usually, after elimination of hyperthyroidism, ejaculation completely normalizes in a few weeks.
Treatment of early ejaculation due to chronic prostatitis is reduced to the elimination of infectious inflammation in the prostate gland. However, it should be remembered that antibacterial therapy is performed only after taking the therapeutic measures for the destruction of purulent congestion in the ducts of the prostate, consisting of clusters of glued-together leukocytes.
In some cases, the anti-PE spray may be used. Sprays should be applied on the penis before sexual intercourse. They contain substances that act like slight anesthetics.
Premature ejaculation treatment: SDN surgery
In situations where the patient wants to have a lifelong effect to increase the duration of the sexual intercourse, a premature ejaculation treatment surgery can be recommended – a selective dorsal neurectomy (SDN). SDN is used primarily in Asian countries, however, it has gained increased interest in Western ones (as stated in the study by Du Geon Moon, 2016). The SDN method consists in isolating, crossing and subsequent sewing of the rear nerves of this organ, which determine sensitivity. Within the first month after the operation, patients notice complete anesthesia of the glans penis, but soon the sensitivity is partially and stably restored, which allows for a long-term sexual intercourse. The method requires special micro-surgical equipment and the appropriate qualification of the surgeon, which determines the cost of the operation. As with gel implantation, it should be borne in mind that in patients with ejaculation occurring before the introduction of the penis into the vagina, the use of selective dorsal neurectomy is ineffective.
How to control premature ejaculation with special techniques
People that don’t want use premature ejaculation pills can try special anti-PE techniques developed by sexologists. There are two main anti-PE methods:
1) “Start-stop”. During sex, when a man feels the onset of orgasm, the partners should begin to move more slowly, until the urge to ejaculate disappears.
2) “Squeeze”. When a man feels almost ready for orgasm, he should say to a woman to squeeze the penile organ near the glans with thumb & index fingers. A woman should hold this squeeze for 10 seconds or until the urge to orgasm disappears. In some cases, this technique can cause painful or discomfort sensations.
Premature ejaculation treatment: home remedies
Using home remedies is popular among males because many of them don’t want to visit a health care professional with such a delicate problem. Folk remedies can be in the form of infusions, concoctions of natural herbs and roots. Such remedies can easily be bought in any pharmacy. Brewed with hot water, they are usually taken a couple of times a day. Instructions for using them are on the packages, and for different remedies the course of treatment can be different. It should be noted that the use of home remedies for the treatment of PE can be unsafe for your health. Therefore, it is strongly recommended to consult a doctor and agree on treatment methods with him.
How to prevent premature ejaculation
If you learn how to prevent the disease, you will not have to learn how to fix it and how to control it. However, many physicians are convinced that PE is not a real disease, and therefore its specific prevention as a disorder is completely absent. Unfortunately, this phenomenon is quite frequent. Prevention of the development of sexual disorders, including PE, can largely depend on the lifestyle of a particular man and his habits. In this respect, factors such as the general health of a particular man, the functioning of his nervous system, the principles of nutrition, sleep and rest, the rhythm of life, and relationships with a partner can be important.
